The Care Navigator Service has been operating sucessfully 
in the East of Kent since 2008.
We are now delighted to announce that it has been expanded into
West Kent.
At various times in our lives, we all face change. Some of us need help to decide how to deal with these changes. Your local Care Navigator offers free impartial advice aimed at people over 50 who feel they need extra support to remain independent in their own home.
How the service works
We can arrange for a Care Navigator to meet you, usually in your home, to discuss your needs in a confidential and non-judgmental manner. This can include things such as
- Benefit awareness
- Care service provisions
- Home maintenance
- Housing choices
- Safety in the home
- Self assessments
- Adaptations and Equipment
- Help in planning your support
The Care Navigators will look into the issues you have discussed and help you find the best way forward, offering you some options, where appropriate. We can support you to make contact with other professional organisations and agencies.
Care Navigators cover Dartford, Gravesham, Maidstone, Tonbridge and Tunbridge Wells.
The Care Navigation project is managed by Voluntary Action within Kent, in partnership with Carers First.
